Draft a QRC change notice letter
This tool helps you draft a plain-English notice letter to an insurance adjuster and your current QRC when you want to select a new QRC. In Minnesota, there is a 60-day window to choose a different QRC without insurer permission — and the clock starts from the date the rehabilitation plan (R-2) is filed, not your injury date and not the date you first met the QRC.
In Minnesota, you can usually change your Qualified Rehabilitation Consultant (QRC) within 60 days after the rehab plan (R-2) is filed by giving written notice.
- • Based on Minn. Stat. § 176.102 and Minn. R. 5220.0710
- • Generates a copy/paste letter and a downloadable receipt
- • Nothing is saved unless you choose to
