R-2 (Rehabilitation Plan)

The form documenting the initial rehabilitation plan in a Minnesota comp claim; its filing starts the 60-day window to change QRCs.

The R-2 is the rehabilitation plan itself: the form your QRC files setting the return-to-work goal, the services to be provided, and the expected timeline and cost. It follows the initial rehabilitation consultation and is the founding document of the rehab side of your claim under Minn. Stat. § 176.102.

Its most important side effect is a deadline: filing the R-2 starts your one-time, 60-day right to change QRCs without needing insurer agreement or a judge. Workers who feel their QRC was chosen by (and works comfortably with) the insurer have exactly that window to act freely.

Read the R-2’s goal line carefully. "Return to work, same employer, same job" versus "return to work, different employer" versus retraining framings shape everything the QRC does afterward, and whether the plan is aiming at genuinely suitable gainful employment or just file closure.
